Medical Insurance & Payment Methods

On Your Initial visit, please bring with you a referral notice from your primary care physician unless prior contact has been established with our office. Also bring with you all pertinent medical information.

We will be pleased to file your insurance claim for you. You will be asked to pay any deductible amount or copayment at the time of your first visit or to pay the full amount if you are a self-paying patient. Visa and MasterCard are accepted.

If you anticipate difficulty in paying, please call our office ahead of time to arrange a meeting with our financial counselor to work out a payment plan.

Should your insurance company not pay for any anticipated coverage, you will be billed for the uncovered amount and will have thirty days to make payment.
